(Chapter 30, 33, 1606, and 1607) Submit this form when applying for benefits for the first time or when changing chapters.
22-1995 Request for Change of Program or Place of Training
(Chapter 30, 33, 1606, and 1607) Submit this form when transfering schools or if you have already applied for benefits before.
22-5490 Dependents' Application for VA Education Benefits
(Chapter 35) Submit this form when applying for benefits for the first time.

22-1990e Application for Family Member to Use Transferred Benefits
Chapter 33 Dependents who have been approved for transfer of entitlement.
This form is used as an appeal to the Department of Veterans Affairs.
ARC Veterans Add Class Request
Use this form when you make changes to your schedule after submitting your initial enrollment to the VA Office.
ARC Veterans Finals Verification Letter
Use this form after completion of your courses (at the end of each semester) or if you stop attending any of your courses which result in an "F" grade.

ARC Individualized Math Course Level Verification
This form is used to verify the math level of an individualized math 1000 course. This form must be signed by your instructor.
