VRC and VA Forms

 

A student’s signature is required to process any of the following forms. Please submit forms to the Veterans Resource Center. For any questions please contact our office at 916-484-8135.

 

ARC Veterans Cover sheet This form is used as a coversheet for new students submitting VA paperwork for the first time.

Statement of Student Understanding and Responsibilities This form is used as part of the initial VA paperwork for new students. Please read through carefully, as this is your responsibilities as a student in order to use your VA educational benefits.
 

22-1990 Application for VA Education Benefits (Chapter 30, 33 & 1606) Submit this form when applying for benefits for the first time or when changing chapters.

 

22-1995 Request for Change of Program or Place of Training (Chapter 30, 33, 1606 & Transfer of Entitlement Program) Submit this form when transferring schools or if you have already applied for benefits before.

 

22-5490 Dependents' Application for VA Education Benefits (Chapter 35 & Fry Scholarship) Submit this form when applying for benefits for the first time.

 

22-5495 Request for Change of Program or Place of Training Survivors' and Dependents' Education Assistance (Chapter 35 & Fry Scholarship) Submit this form when transferring schools or if you have already applied for benefits before.

 

22-1990e Application for Family Member to Use Transferred Benefits (Dependents Chapter 33-Transfer of Entitlement Program) Submit this form when approved for transfer of entitlement.

 

22-4138 Statement in Support of Claim This form is used as an appeal to the Department of Veterans Affairs.

 

Cal-Vet Fee Waiver Application This form is for dependents of Veterans only.

 

ARC Veterans Academy Cover sheet This form is used as a coversheet for students who are enrolled in academies. (Police, Sheriff and CHP)

 

Do Not Drop Request This form is for recipients of VA education benefits whose tuition/fees are being paid either in full or a percentage of by the Department of Veterans Affairs. The purpose is to prevent student from being dropped for non-payment.
